Session Description:
The panel discussion on “The Future of Primary Care” explores the evolving landscape of primary health care, addressing key trends, challenges, and opportunities that will shape its future. Our panelists will share their insights and perspectives, delving into emerging trends in primary care, strategies to improve access and equity in health services, and the changing dynamics and needs of the primary care workforce.
Session Objectives
Explore emerging approaches to improving access to primary care services, particularly for underserved and marginalized populations.
Describe primary care workforce trends, including the integration of advanced practice providers and interdisciplinary teams.
Identify future models of integrated care between primary care, specialty care, and social services.
Hear select CHIP presentations and network with the graduating cohort around their CHIP projects.
Session Objectives:
Describe at least one California Health Care Improvement Projects (e.g. impact of the project).
Identify lessons or methods that can enhance systems-based practice.

Session Description:
Panelists will describe the intersection of climate change and health equity and discuss how environmental factors disproportionately affect vulnerable populations and exacerbate health disparities. The session will consider the health effects of climate change, the role of health systems in mitigating climate impacts, and community-based approaches to enhance climate resilience and equity.

Describe the role of data and analytics in tracking and addressing climate health disparities.
Explore how health care systems can address the health impacts of climate change and integrate climate resilience into operations and patient services.
